WAIVER FORM: |
All participants must sign the Individual Sport Waiver Form prior to competition. The waiver form must be submitted to SSG staff at check-in. Participants under the age of 18 must have a parent or guardian's signature. Click here to download and print the 2017 SSG Individual Waiver Form. |
